INSURANCES
UPARHS is a participant in some of the insurances in our area.
The listing of participating programs can be found on this website under the sidebar entitled INSURANCES. If
you do not see your insurance listed, please ask us.
Included in the list of insurances is MICHIGAN MEDICAID for both Medical and Dental.
For clients living in Wisconsin, UPARHS also accepts WISCONSIN MEDICAID for Medical and Dental or often
referred to as Badgercare or ForwardHealthWI.
SELF-PAY
Those who have the means and have no need of financial assistance are welcome to participate in both Medical
and Dental Services. Payment, whether full or partial, will be expected at the time of service.
SLIDING FEE PROGRAM
This program is intended to assist those who have modest means and thus are able to receive an adjustment
to their medical or dental charges. The amount of the discount is contingent on the financial status of the
person. A minimum co-pay of $10.00 at the least for medical services and $20.00 at the least for dental
services is expected at the time of treatment.

PAYMENT PROGRAMS
If a person has no insurance and is not qualified for the Sliding Fee Program, a payment plan that is
acceptable to both the responsible person and UPARHS can be negotiated with the site manager or the office
assistant.
Since it is the policy of the Association to care for people who are in need of health care, the health
centers must not be considered a "free clinic". For that reason, failure to pay for services rendered will
be taken seriously and pursuit of payment will be undertaken.
CARECREDIT
UPARHS participates in the Care Credit Program.
