The mission of the Upper Peninsula Association of Rural Health Services is to provide high quality primary health care with respect and compassion to all in need.

Our Vision:

Our vision for the UP Association of Rural Health Services is to be a major health provider in the Upper Peninsula of Michigan which offers a coordinated and expanded system of primary care including medical, oral health, nutrition, vision, preventive care, mental health and pharmacy.

Dental Providers



BRIAN CARMODY, D.D.S.

Dr. Brian Carmody, DDS, earned his dental degree from the University of Detroit-Mercy School of Dentistry in Detroit, MI. He also holds a Bachelor’s degree in biomedical science from Grand Valley State University in Allendale, MI. He performed his student rotation at Cherokee Indian Hospital in Cherokee, NC. He has volunteered for the “Give Kids A Smile Day,” a program that provides basic dental care to children who might otherwise go without, and has also volunteered for a program that performs free screenings for braces.

He is a member of the Michigan Dental Association, and the Christian Medical and Dental Association. Dr. Carmody is a native of Jackson, MI. He and his wife reside in Menominee.

Dr. Carmody has been a dentist of the U.P. Association of Rural Health Services, Inc. since July 2010. He provides dental care to patients primarily at the Northern Menominee Health Center South in Menominee.


MATTHEW HOSTETLER, D.D.S.

Dr. Matthew A. Hostetler, DDS, a native of Fort Wayne, IN., is a graduate of the Indiana University School of Dentistry. While earning his degree he also volunteered at Head Start programs in Indianapolis where he taught children proper dental hygiene skills and provided education about oral health.

After graduating, Dr. Hostetler practiced with the Glassley Dental Group in Fort Wayne. He is a member of the American Dental Association.

Dr. Hostetler became an employee of the U.P. Association of Rural Health Services, Inc., in 2010. He cares for patients primarily at the Dickinson-Iron Dental Center in Crystal Falls. He also cares for patients at the Ewen Medical-Dental Center in Ewen, and the Northern Menominee Health Center in Spalding as needed.


KELSEY PATTON, D.D.S.

Dr. Kelsey Patton, DDS, a native of Baraboo, WI, is a 2010 graduate of the Indiana University School of Dentistry in Indianapolis, IN.

During her school years she served a dental externship at a private practice in Madison, WI, and was an oral surgery assistant at the Oral Surgery Center in Baraboo. Dr. Patton was a member of the Indiana University School of Dentistry Kids Club where she worked with children providing oral exams, fluoride treatments, and education about proper oral hygiene and dental care.

Dr. Patton also participated in an international service learning trip to Honduras where she performed a variety of dental procedures for residents of the third-world nation who rarely, if ever, have access to professional dental care.

Dr. Patton has been employed by the U.P. Association of Rural Health Services, Inc., since 2010. She provides care for patients primarily at the Northern Menominee Health Center in Spalding, and rotates to other sites as needed.


CHRISTOPHER C. MAKI, D.D.S.

Dr. Christopher Maki attended the University of Michigan, School of Dentistry in Ann Arbor, MI where he received his DDS. degree in 1988. For the next year he was an associate with Dr. Stan Gresham, Ironwood, MI and then from 1990 to 1993 he was Dentist, Consultant and Director of Mobile Dental Project for the Western Upper Peninsula District Health Department.

From 1993 to 1996 he was Dentist/Director of Jackson Area Dental Clinic. Since 2000 Dr. Maki is in private practice in New Lothrop, MI. He also serves as a "locum tenens" dentist for UPARHS as needed. Most often Dr. Maki works at West Mackinac Dental Clinic in Engadine, MI.

 

REGISTERED DENTAL HYGIENISTS


ELLEN MOILANEN, RDH

Ellen attended the Mott Community College in Flint, MI, receiving her degree as a Registered Dental Hygienist in 1982. Prior to coming to work for the Association in August of 1995, she worked in various dental offices and for the state as a Dental Hygienist. Ellen serves as dental coordinator and performance improvement/JCAHO coordinator for the U.P. Association of Rural Health Services, Inc. She also coordinates the Head Start childhood fluoride varnish and dental sealant program.

CHANTELLE EICHHORN, RDH

Chantelle attended Ferris State University, Big Rapids, MI, receiving her degree as a Registered Dental Hygienist in 2002. Prior to coming to work for the Association in June of 2005, she worked at Knudsen Dentistry in Escanaba, and Lakeview Dental Associates in Lakeview, MI.

KIMBERLY BROWNING, RDH

Kimberly attended Baker College of Port Huron, MI, receiving her degree as a Registered Dental Hygienist. She has worked as a dental hygienist at the Sawyer Dental Center near Gwinn since 2007.

HEATHER LYNN SEYMOUR, RDH

Heather attended Northeast Wisconsin Technical College in Green Bay, WI, receiving her degree as a Registered Dental Hygienist in 2005. She joined the Association in 2006.
